How Do I Get Started?

  • Phone our 24 hour Answering Service, (706)860–8860, and let them know you would like a return phone call from one of our therapists. They will ask for your name & your phone number.
  • A therapist will return your call and an appointment can be arranged at that time.
  • Fill out the Client Information Form and bring it to your appointment.
  • Read, sign, and initial the Informed Consent Form (initialing that you have also read the “Biographical Information” sheets pages on the consent form). Bring the Informed Consent From to your appointment.
  • Bring your Health Insurance card and/or your military ID
  • Some health insurance companies require that the member phone them prior to the appointment. If you do this, bring to the appointment the pre-authorization/certification number they assign you.
  • Remember to bring to the first appointment:
    1. Client Information Form
    2. Informed Consent Form
    3. Health Insurance card and/or your military ID
    4. Pre-authorization/certification number (if applicable)
